What to Expect at Your First Visit
When you arrive for your first visit, please be prepared to complete all insurance and health information forms that will allow us to begin your thorough dental examination. We will ask you to fill out several forms that will get you acquainted with our office. For your convenience, let us know if you’d like us to email you those forms so that you can fill them out at home and send them back to us with one click.
If you have dental insurance, please be sure to bring your insurance card. This will enable us to give you an estimate of your costs.
Following a thorough consultation and exam, we will work with you to develop a personalized treatment plan to meet your specific needs and desires. We encourage you to ask questions. We’ll be glad to discuss all your treatment options with you.

We believe that good dental care begins with open communication. We promise to speak candidly with you about our recommendations, any alternative treatments available, and any symptoms present that may require treatment from a specialist. Working together, we can achieve a true partnership, with one common goal – keeping your smile beautiful!
New Patient Exam and Consultation
If you’re a brand new patient to our office, first of all, welcome! We are so happy you chose Creating Smiles to take care of your teeth needs. We wanted to help prepare you for your first visit. We want to get to know you and your current state of oral health. We are big believers in knowing all about the progression of your teeth, so to get a better idea of where we’re starting, we take x-rays of every area of your mouth. Once we gather all the images, we examine them and let you know what areas appear healthy and which areas spark a bit of concern. This process takes around an hour, and if you have a cleaning scheduled afterward, please consider that timing as well. We hope that when you leave your appointment, you understand our passion for taking care of your teeth and preventing any future problems while we call you our patient.
Online Forms
At Creating Smiles Family and Cosmetic Dentistry, we offer patient forms online so you can complete them in the convenience of your own home or office. Fax us your printed and completed forms or you may bring them during your visit.

Insurance
Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts.
Please be prepared to give us your insurance information on your initial phone call so that we can call your insurance company for a courtesy benefit check prior to your initial appointment.
When you first visit our office, bring your current insurance card with you. If you change insurance companies or join another employer’s plan, please be sure to let us know. If an insurance referral is necessary, please bring the completed form with you to your appointment.
We are a preferred provider with Advantica, Aetna, Ameritas, Blue Cross Blue Shield of Illinois, Cigna, Delta Dental, DenteMax, Guardian, Humana, MetLife and United Concordia.
We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your policy has not paid within a reasonable timeframe. If we are not contracted under your insurance, you will be given a copy of services and charges to file with your insurance claim form.
